29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about June 17, 2015, the employer failed to ensure that effective guarding was in place and utilized on the Toyokoki Hyb-100 hydraulic press brake. The foot pedal is not affixed in place and is easily movable by employees. Light curtains in place on the press brake were disabled and not utilized by operators.
