Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place fo employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by logs while loading: a) Evans Sale, Dempsey Creek - On or about January 9, 1990 and at times prior thereto, employees bucking on landings were in close proximity to loading operations. Serco loader was damaged with rear window replaced by cardboard, restricting vision of loader operator. Among other methods, one feasible and acceptable a safe distance from correct this hazard is: 1) Require employees to maintain a safe distance from loading and skidding operations. 2) Repair rear cab window on Serco loader. 3) Implement formal written job training and accident prevention program to include: a) Orientation of new employees. b) Monthly safety meetings. c) Frequent visual observation of work practices.
