Text For Citation: 01 Item/Group: 003 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Mid-America Millwright Service, Inc. employee(s) were working at Ox Town Cattle Feeders Inc. and the equipment on which the employee(s) were working was not identified as unsafe to operate. On or about June 10, 1996, Mid-America Millwright Service, Inc. employee(s) were exposed to crushing injuries while performing work on the feed mill elevator conveyor belt and pulley in that tags or locks were not affixed to the energy control devices(s). Among other methods, one feasible and acceptable abatement method to correct this hazard is to identify the equipment unsafe to operate by using locks and/or tags or some other equivalent method. IN THE ALTERNATIVE 29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about June 10, 1996, Mid-America Millwright Service, Inc. employee(s) working at Ox Town Cattle Feeders Inc. were exposed to crushing injuries in that lockout or tagout devices were not affixed to the energy isolating devices for the feed mill elevator leg prior to the employee(s) initiating work on the leg conveyor belt and pulley. Locks and/or tags were not affixed to the energy isolating device(s).
