(c) Worksite - Employees entered and worked in an unprotected trench, identified as grave opening # 25-Y-103, to install components of the "Grave Bar" shoring system and level the bottom of the trench. The spoil pile, measuring approximately 6' - 8' H x 18' in diameter, was placed directly on the east edge of the trench and there were no restraining devices in place to prevent the excavated materials from falling back into the grave opening on, or about May 12, 2015.
