29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames shall have a maximum spacing between rungs of 16 3/4 inches (43 cm): a. Along the exterior wall, single tier prefabricated frame scaffolding with rungs spaced 20 inches apart were used by employees as a ladder to gain access to the working level of the scaffolding, exposing the employees to 5 foot fall hazard, on or about 04/09/2015.
