29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a. On or about March 3, 2015, the storage mezzanine above in the processing building did not have complete guardrails. Employees stowing and retrieving packaging materials were exposed to a 9 feet fall hazard from the mezzanine floor to the concrete processing floor due to the nearly 35 feet of unprotected edges of the mezzanine. b. On or about March 3, 2015, employees accessing the rooftop via the catwalk above the blast freezer were exposed to an 11 feet fall hazard due to incomplete guardrails along the catwalk. c. On or about March 3, 2015, employees working from the condenser platforms and catwalks were exposed to an 8 feet fall hazard at the two floor openings between the three condenser banks that were not covered and did not have guard rails. d. On or about March 3, 2015, employees accessing the rooftop ice machines via condenser catwalks were exposed to a 15 feet fall hazard from the unprotected edge of the roof by the condensers that did not have guardrails. e. On or about March 3, 2015, employees working at the rooftop ice machines were exposed to a 20 feet fall hazard from the upper roof in that there were no guardrails.
