29 CFR 1910.219(k)(1): Clutches, cutoff couplings, or clutch pulleys had projecting parts which were not enclosed by a stationary guard. a) On or about April 9, 2015, in the Forge shop located at 53 Freedom Rd., Painesville, Ohio 44077, the operator of the 150 ton Maxi Press was exposed to the unguarded rotating clutch brake pulley .
