29 CFR 1910.119(j)(4)(ii) Inspection and testing procedures shall follow recognized and generally accepted good engineering practices: a)Mahantango Site, O Cellar, the ammonia to glycol tube and shell heat exchanger had distorted/broken outer aluminum jacket, disturbed insulation, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. b)Mahantango Site, ammonia / carbon dioxide engine room, piping and appurtenances to the ammonia to glycol heat exchanger had missing insulation, visible corrosion, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. c)Mahantango Site, main ammonia engine room, compressor #5 ammonia had an inlet elbow assembly which had no insulation, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. d)Mahantango Site, main ammonia engine room, piping and tee as part of the pump receiver vessel installation which was located adjacent to compressor #6 had missing insulation, ice deposits, and the employer did not document that the inspection and testing procedures complied with recognized and generally accepted good engineering practices, on or about June 18, 2015.
