Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were operating a freight elevator with its inner doors open exposing employees and other employers employees to hazards of being caught between the elevator car platforms and structural members of the building and/or shaft walls resulting in crushing, amputation or death and/or otherwise exposed to in- juries from improper elevator use. Employees were not trained on how to safely operate the freight elevator and to ensure all safety devices are in an operable condition. Among other meth- ods, one feasible and acceptable method(s) to correct this haz- ard is to: (1) To train all employees operating the freight elevator on the controls and safety devices of the freight elevators on what their function and purpose is as well as the safe placement of passengers and safe operating prac- tices.
