Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing of likely to cause death or serious physical harm to employees in that roof attachment points used to anchor the primary support system and lifeline of a single point suspension system were not capable of supporting the loads imposed on it: a) Academy House, 1420 Locust Street, Phila, Pa. 19102 - the roof attachment point used by employee to anchor hsi primary support and lifeline failed resulting in a fall 38 stories (fatality occurred 12/12/90). Observed 12/12/90 NOTE: One useful and feasible means of abatement, among others, would be to load test the attachment points for the primary support and lifeline prior to the start of the job.
