29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Maintenance Shop, on or about 2/3/15: Extension cord providing current to the fuel pump was not grounded in that the ground pin was missing from the plug end. b) Pallet Shop, on or about 2/3/15: An extension cord was not grounded in that the ground pin was missing from plug end. Abatement certification must be submitted for these items.
