29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about January 21, 2015, at the facility located at 106 W 31st St, Independence, MO., the employer did not ensure all Powered Industrial Truck Operators had successfully completed the training required by this standard. There were no records certifying each operators date of training, date of the evaluation, and the identity of the person(s) performing the training or evaluation. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
