29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: Lebanon Seaboard Corporation had an employee exposed to amputation hazards and physical injuries, while performing service and maintenance activities on the 05 Line Mixer during the second full week of January 2015. Two employees performed the service and maintenance activities but only one applied an energy control device.
