Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the hazards of being caught in or crushed while working in and around machinery and equipment: a) In Plant 5, 14 inch MIll, in the Old Piler holding bed, an employee was sweeping the floor and the main power disconnect was not turned off and locked out. b) In Plant 1, Nail Department, an employee was clearing a jam on the conveyor for No.1 5 pound machine, and the main power for the conveyor had not been turned off and locked out. Among other methods, one feasible and acceptable abatement method to bleeding-down, etc. out procedure such as that stipulated by ANSI Z244.1-1982 Sections 4 and 5, American National Standard of Minimum Safety Requirements for Lockout/Tagout of Energy Source for Personnel Protection. Essential has been completed. NOTE: Test shall only be performed after the following procedures: 1) Require lockout before and maintenance, inspection, clearing, adjusting or servicing of machinery or equipment that requires entrance into or close contact with the machinery or equipment. 2) Required that each exposed worker lockout the energy isolating device with his personal lock. 3) Maintain available operation and maintenance manuals on all machinery or equipment to be serviced. Assure provision of appropriate manuals and information to affected workers before work is begun. 10. Enforce all of the provisions of the program. 6) Test machinery or equipment to ensure that the energy sources have been effectively disconnected or isolated. As a precautionary measure return the operating controls to the "OFF" position after the test has been completed. NOTE: Test shall only be performed after ensuring that no personnel are exposed in the machine or equipment area. 7) Remove locks only after ensuring safety of all exposed employees. 8) Train employees responsible for the machinery or equipment lockout in all elements of the program. The training shall be repeated as frequently as necessary to maintain management and work proficiency and compliance with the procedure. 9) Evaluate the program periodically to ensure usage and adequacy of the program. 10) Enforce all of the provisions of the program.
