29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his environment to control or eliminate any hazards or other exposure to illness or injury: a) IVO Port of Wilmington - The employer did not instruct the employees working on Derrick 15, specifically the mates/crane operators, how to recognize or avoid the hazardous condition of gross overload during a lift. This was observed on December 2, 2014 while retrieving an anchor cable, resulting in the derrick conducting the lift overturning and causing the drowning death of one employee.
