29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire: In front of Production Office and next to Grisly Pit, On or about November 26, 2014, employees were exposed to burns, when 8 full and 8 empty oxygen cylinder were stored together in a storage rack and not separated from the 5 full and 3 empty fuel cylinders containing propylene, when the employees replace the used with full cylinders.
