29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) At the recycling facility in Unadilla, GA: On or about August 5, 2014, and at times prior thereto, the employer exposed employees to fire hazards including death, in that a pedestrian exit-route door at the south-end receiving dock required excessive force to open from the inside, impeding quick-building egress.
