29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Equipment in the Roll Forming Department, including roll forming machines SL6 and SL8, is being set-up without isolating all hazardous energy sources and affixing a lockout or tagout device to the energy isolation device(s). (b) Maintenance activities of a short duration are sometimes conducted without affixing a lockout or tagout device to the energy isolation device(s).
