29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: a. North side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which had a single scaffold plank measuring approximately 2X12, while installing the cinder blocks to the wall. b. East side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which had a two scaffold planks, a 2X12 and a 2X6 separated by more than 4 inch, on the top level and only one at other levels, while installing the cinder blocks to the wall. c. South side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which had only two scaffold planks, a 2X10 and a 2X6, separated by more than 4 inch, on the top level and at other levels only one 2X10, while installing the cinder blocks to the wall.
