29 CFR 1926.1208(b): (b) Properly use equipment as required by �1926.1204(d); a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to ensure that atmospheric monitoring equipment used to evaluate the permit-required confined space was properly calibrated before use. Specifically, the four-gas meters provided by the Cincinnati Zoo had not been bump tested to verify their accuracy before employees entered the confined space for gas line removal and replacement. As a result, employees relied on unverified atmospheric monitoring data, increasing the risk of undetected hazardous atmospheric conditions within the confined space.
