29 CFR 1926.1204(h): (h) Designate each person who is to have an active role (as, for example, authorized entrants, attendants, entry supervisors, or persons who test or monitor the atmosphere in a permit space) in entry operations, identify the duties of each such employee, and provide each such employee with the training required by �1926.1207; a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to designate individuals responsible for active roles in the permit-required confined space entry, including authorized entrants, attendants, entry supervisors, and personnel responsible for atmospheric testing and monitoring. Additionally, the employer failed to identify the duties of each individual involved in entry operations and did not provide the required training for employees entering the confined space. As a result, employees entered a permit-required confined space without clear role assignments or the necessary training to recognize and respond to potential hazards, increasing the risk of exposure to hazardous atmospheric conditions and uncontrolled gas line movement.
