29 CFR 1926.1204(l): (l) Develop and implement procedures (such as closing off a permit space and canceling the permit) necessary for concluding the entry after entry operations have been completed; a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to develop and implement procedures for concluding entry operations after work inside the permit-required confined space was completed, including closing off the space and canceling the entry permit. As a result, there were no documented procedures to ensure the permit space was properly secured after entry operations ended.
