29 CFR 1910.146(e)(1): The employer did not document the completion of confined space entry measures required by paragraph (d)(3) of this section by preparing an entry permit. a) On or about August 11th, 2018 or prior thereto: Entry was conducted into a 8000 gallon 1988 Beat Tanker DOT classified 306 trailer to weld a baffle. The employer did not evaluate the respiratory hazards inside the tanker prior to entry, and document this assessment with an entry permit. NOTE: Abatement certification and documentation are required for this item.
