29 CFR 1910.335(a)(1)(v): Employees exposed to the danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion did not wear protective equipment for the eyes or face: (a) On or around March 6, 2018 in the OMCC area of the Galveston Bay Refinery, the employer did not ensure that I&E technicians working at Load Center 46 wore protective equipment for the eyes or face when exposed to electric arcs or flashes from flying objects resulting from electrical explosion when troubleshooting the motor control center for the F35A pump was open.
