29 CFR 1910.146(d)(9): Under the permit space program required by paragraph (c)(4) of this section, the employer did not develop and implement procedures for summoning rescue and emergency services and for rescuing entrants from permit spaces: On or about October 12, 2011, in the facility, employees were required to enter permit spaces, including, but not limited to, ISO containers, during cleaning and maintenance operations. Potential hazards in permit spaces included oxygen deficiency, toxic and flammable/combustible atmospheres from residual (heel) and spot cleaning chemicals. The employer did not develop and implement procedures for rescuing entrants from permit spaces. Local emergency responders (local fire department) were never notified of permit space entry work being performed at the site. Employee(s) (attendants) were not provided with adequate training on rescuing entrants from permit spaces with available retrieval system.
