29 CFR 1910.146(f)(13): The entry permit that documented compliance and authorized entry to a permit space did not identify equipment, such as personal protective equipment, testing equipment, communications equipment, alarm systems, and rescue equipment: a) 4650 Canton Road, Plant 500, Marietta , GA: The employer allowed the contractor employees to work in a permit-required confined space (silo #2) where the entry permit, signed by the host employer, did not identify proper testing and communication equipment. The attendant could not effectively communicate with the authorized entrants while inside the silo to monitor the status or to alert entrants of the need to evacuate the space when a hazard was known or detected.
