29 CFR 1926.1204(k): (k) Develop and implement procedures to coordinate entry operations, in consultation with the controlling contractor, when employees of more than one employer are working simultaneously in a permit space or elsewhere on the worksite where their activities could, either alone or in conjunction with the activities within a permit space, foreseeably result in a hazard within the confined space, so that employees of one employer do not endanger the employees of any other employer; a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to develop and implement procedures to coordinate entry operations with the controlling contractor while multiple employers had employees working simultaneously at the site. As a result, the activities within the permit-required confined space, including the removal and replacement of gas lines, were not coordinated to ensure that the actions of one employer did not create hazards for employees of another employer, increasing the risk of exposure to uncontrolled hazards within the confined space.
