29 CFR 1910.272(g)(3): In the grain handling facility an observer, equipped to provide assistance, was not stationed outside the bin, silo, or tank being entered by an employee: (a) On or about June 10, 2020, and at times prior to that date for the employee who suffered fatal injuries after entering a grain bin with approximately 20,000 bushels of crusted sunflower seeds in Bin 133 at 2147 77th Avenue SE in Pingree, North Dakota. (b) On or about May 21, 2020, and at times prior to that date for employees exposed to potential health and safety hazards while working alone inside grain bins at 2147 77th Avenue SE in Pingree, North Dakota.
