29 CFR 1910.335(a)(1)(iv): Employees exposed to the danger of head injury from electric shock or burns due to contact with exposed energized parts did not wear nonconductive head protection: (a) On or around March 6, 2018 in the OMCC area of the Galveston Bay Refinery, the employer did not ensure that I&E technicians working at Load Center 46 wore protective helmet to reduce electrical shock or burn hazards due to contact with exposed energized parts when electrical panels on the motor control center for the F35A pump were opened.
