29 CFR 1910.146(i)(7): The employer did not ensure that the Attendant summoned rescue and other emergency services as soon as the attendant determines that authorized entrants may need assistance to escape from permit space hazards. a) On or about July 19, 2017 or prior there to, Sulfuric Acid Tank #5: An emergency response was required after entrance into a confined space with concentrated 98% Sulfuric Acid, and the designated Attendant was not supplied with a radio or communication device to summon assistance. NOTE: Abatement certification and documentation are required for this item.
