29 CFR 1926.1204(f): The employer did not provide an attendant outside a permit confined space for the duration of entry operations: a. E E Robinson Park (Level Creek), 800 Peachtree Industrial Blvd. Sugar Hill, GA - An employee was exposed to asphyxiation hazard. The employer did not station at least one trained attendant outside a permit space to perform all attendant's duties. The employer removed the manhole cover and purchased a flash light with the expectation that the employee would break the plain, on or about September 6, 2016.
