Forest City Technologies, Inc. located in Wellington, Ohio: On or about July 22, 2016, and July 25, 2016, the employer did not ensure employees cleaning a storage tank located in Plant 4 were provided with hand protection to protect against contact with liquids and solids from a priming chemical used in the production process.
