29 CFR 1910.146(f)(12): The entry permit that documented compliance and authorized entry to a permit space did not identify the communication procedures used by authorized entrants and attendants to maintain contact during the entry: a) 4650 Canton Road, Plant 500, Marietta , GA: The employer allowed authorized entry into a permit-required confined space (silo #2) and did not identify the communication procedures that would be used by the authorized entrants and attendant to maintain contact during entry. Cell phones were used to maintain contact, which is not an acceptable procedure.
