29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: (a) In the Shop: On or about February 24, 2012, and at times prior thereto, an extension cord in the north east corner of the shop was missing the grounding prong. Note: Abatement certification is required for this item.
