29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Processing room; On or about January 8, 2025, and days prior, energy control application steps such as, machine shutdown [(d)(2)], machine isolation [(d)(3)], and lockout/tagout device application [(d)(4)] were not in place prior to performing covered activities such as but not limited to clearing jams, replacing plastic rolls, and disassembly/cleanup on the Multivac Type R105 (Serial No. 308042) and the Multivac Type R105 (Serial No. 162229) packaging machines.
