29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about November 20, 2024, employees performing servicing and maintenance activities, including setting up the Bradbury Hemmer, were exposed to hazardous stored energy, including but not limited to, electrical and gravity, and the employer did not ensure locks were used by each employee to isolate and control the energy sources.
