29 CFR 1926.1204(j): (j) Develop and implement a system for the preparation, issuance, use, and cancellation of entry permits as required by this standard, including the safe termination of entry operations under both planned and emergency conditions; a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to develop and implement a system for the preparation, issuance, use, and cancellation of entry permits for work inside a permit-required confined space. As a result, employees entered the confined space to remove and replace gas lines without an entry permit, which documents safe entry conditions, authorized personnel, and procedures for terminating entry operations under both planned and emergency conditions.
