29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each isolating device by authorized employees: (a) This violation occurred on or about May 13, 2022, where employees working as machine operators in the production department and did not apply lockout/tagout devices to the Mercury 1 machine, serial number 20210601. Employees were exposed to an amputation hazard.
