29 CFR 1910.146(e)(1): The employer did not document the completion of measures required by paragraph (d)(3) of this section by preparing an entry permit before entry was authorized: (a) Dick's Sporting Good, Inc. at 31 W Flatiron Crossing Drive in Broomfield, CO: On or before March 30, 2021, the employer did not require an entry permit be completed before entry into the permit space. The compactor met the definition of a permit space, and operations employees regularly entered the space while safety hazards were present, but the employer did not ensure requirements for permit space entry were implemented, including developing rescue procedures. This condition exposed employees to crushing hazards.
