29 CFR 1910.178(o)(5): A load engaging means shall be placed under the load as far as possible; the mast shall be carefully tilted backward to stabilize the load. a) On or about June 9, 2020, the employer did not maintain a load engaging means and with the mast tilted backward in that the forklift operator had traveled with an approximately 566 pound steel coil located at the front edge of one fork in the horizontal and hanging vertically from the edge of the fork. Employees working in the area were exposed to struck-by hazards. b) On or about June 9, 2020, the employer did not maintain a load engaging means when employees had to turn a load placed on the winder backwards. Employees were required to hold two approximately 566 pound steel coils upright while unloading and loading the coils on the forks. When the forks were lowered while loading the coils, the coils fell onto an employee.
