29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: a. Extrusion line 1-8 - On or about June 12, 2019, Employees exposed to struck-by hazard while performing service and maintenance without locking out the pneumatic energy source on the equipment. The employer did not ensure that all energy sources were locked out prior to employee performing service and maintenance on the equipment.
