29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. On or about March 21, 2018, the employer failed to ensure employees performing maintenance on the plasma cutter were trained on lockout tagout. Employees were exposed to crushing hazards. b. On or about April 5, 2018, the employer failed to ensure employees cleaning the powder coat wash rack were trained on lockout tagout. Employees were exposed to an approximate 2500 psi water pressure.
