29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work. On or about December 4, 2017 and at times prior thereto, the employer did not ensure employees who disassembling, moving, and erecting a scaffold were trained by a competent person to recognize the hazards associated with the type of work. This exposed employees to a fall hazard greater than 10 feet to the concrete floor below.
