29 CFR 1910.1030(d)(4)(iii)[B](1): Regulated waste was not placed in containers which met the requirements of 29 CFR 1910.1030(d)(iii)[B](1)(i) (iv): On or about October 26, 2017, the employer did not ensure that skin, tissue, and blood removed from the Rockford Manufacturing Group wire feed coil machine (serial number 10816-1312) of the #20 cold former line and surrounding areas following an amputation incident occurring on October 26, 2017 was discarded in appropriate waste containers. The removed material was placed in a regular waste container. The waste container was not closable, constructed to contain contents and prevent leakage, and labeled as a biohazard.
