29 CFR 1910.146(h)(1): Entrants did not know the hazards that may be faced during entry, including information on the mode, signs or symptoms, and consequences of the exposure. a) On or about July 19, 2017 or prior there to, Sulfuric Acid Tank #5: Entrants into the confined space were not trained in the slippery nature of concentrated 98% Sulfuric Acid, nor the hazards of entrance into a confined space by an inverted position without an acid hood, nor the hazards of not having taped PPE seams to protect employees from acid contamination. NOTE: Abatement certification and documentation are required for this item.
