29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. The employer is not training employees authorized to perform repair and maintenance work in all required elements of lockout/tagout, such as machine operators who were not trained on applying locks and tags when replacing pins, changing screens, and replacing squeegees. Work required entering machine areas where moving parts were protected by guard doors, and where alternative safeguards were in need of repair, such as loose emergency stops, and did not always function, such as guard door safety interlocks. b. The employer is not training machine operators authorized to perform repair and maintenance work on isolating electrical energy at the machine disconnect. c. The employer is not training employees authorized to perform repair and maintenance work on the magnitude of energy sources, such as electrical voltage and air pressure. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s). 29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of energy control procedures: The employer is not training machine operators on lockout/tagout, including the purpose of energy isolation, what energy sources needed to be isolated, and what work required energy isolation by authorized employees to perform lockout/tagout.
