29 CFR 1910.218(d)(2): The quick closing emergency valve(s) on the steam hammer(s) were not closed and locked in the off position while the hammer was being adjusted, repaired, or serviced, or when the dies were being changed: 1. On or about November 25, 2014, the employer did not ensure the steam powered forging hammers steam control valve was locked in the off position while the Erie 12,000lb steam hammer, known as 2236, was being repaired. 2. On or about November 25, 2014, the employer did not ensure the steam powered forging hammers steam control valves were locked in the off position while the dies are being changed. Employees change the dies of approximately nine steam powered hammers on a regular basis.
