29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification: a. On or about October 7, 2014, yard, the employer did not require that a Clemco abrasive blasting respirator be used in compliance with the conditions of its certification as follows: 1. Clemco air hose was not used with a Clemco abrasive blasting respirator. 2. Reversed supplied air hose and respirator air hose connections to the CPF filter 3. Supply breathing air pressure at CPF regulator not set in accordance with manufacturers manual Employees wore the respirator during abrasive blasting using crystalline silica containing abrasive which exposes employees to crystalline silica hazards.
