29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage. On or about August 27, 2014 at 2345 Murphy Blvd., employees performing set-up and tool changes do not perform LOTO, the equipment includes, but is not limited to the following: a. E-18, employees performing set up and tool changes on the E-18 are exposed to an amputation hazard. During these operations the employees seldom turn the machines off to perform setup and tool changes. b. E-23, employees performing set up and tool changes on the E-23 are exposed to an amputation hazard. During these operations the employees seldom turn the machines off to perform setup and tool changes. c. P-17, employees performing set up and tool changes on P-17 are exposed to an amputation hazard. During these operations the employees seldom turn the machines off to perform setup and tool changes.. d. P-18, employees performing set up and tool changes on the P-18 are exposed to an amputation hazard.
