29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize those hazards: a) At the jobsite, on or about 4/26/12: Employees were accessing the roof from the top platform of a scaffold by using a 4 foot step ladder that was leand, unopened, against the eave of the house, did not extend at least three feet above the upper surface, and without having been trained on the use of ladders. Abatement certification must be submitted for this item.
